LEARN MORESINGULAR CARE delivers your hemodialysis treatments in the convenience of your own home, in a more comfortable environment, by licensed and experienced dialysis nurses. EVERY VISIT. EVERY TIME.
We provide a complete and all-inclusive dialysis treatment. A monthly dietary counseling is provided by a Registered Renal Dietitian. All personal needs and concerns are further addressed by our Renal Social Worker.
SINGULAR CARE does not require you to see different doctors. We encourage that you retain your own nephrologist. Your personal nephrologist will manage your care with SINGULAR CARE.
You will be furnished with your own hemodialysis machine set-up. The dialysis system will be a single use and remains in your home. We will complete the preventive maintenance, regular servicing of all dialysis equipment.
All necessary nursing and dialysis supplies are provided.
And Yes! Medications like Epogen, Heparin, Venofer, Zemplar, IV antibiotics ordered by your nephrologist are also provided and administered by your dialysis nurse in your place of residence.
Monthly and mid-month laboratory will be collected from your home and results will be communicated to your nephrologist.
Monthly patient conference will be held with you and your health care team.
Biohazard waste disposal pick-up is scheduled regularly.
