What is staff-assisted home hemodialysis?

Home hemodialysis is given in the patient’s home, patient’s room in a nursing home or skilled nursing facilities, rendered by a licensed dialysis nurses under the supervision of your preferred nephrologist. Dialysis treatment is given three times a week over 3-4 hours in duration.

Dialysis Care and treatments are all inclusive with dietary counsel and social worker support. Monthly and mid-month blood samples are drawn and collected and sent to the laboratory from home.


Will my own nephrologist still take care of me?

Yes, SCHHS and your own preferred nephrologist will work hand in hand to provide you with the best quality care home hemodialysis can offer. SCHHS advocate for continuity of care.


What about the equipment and supplies that I will need?

Singular Care will provide all necessary equipments and supplies needed for your home hemodialysis treatment. We have highly trained bio-med technicians that can set up and maintain the equipment to ensure its optimum performance for quality care. These equipments will stay through out the duration of your prescribed home hemodialysis treatment.

Can I afford staff-assisted home dialysis?

Staff-Assisted home hemodialysis, including equipment and supplies, are covered by insurance. We provide assistance in securing approval for Medicare patients with secondary insurance and accept option for self-pay patients. Contact us by filling out the prospective patient intake form.

Who will administer my treatment and take care of me?

Our diverse team of professionals will provide for your care working closely with your nephrologist. We personalize your care by paying close attention to specific needs and establish a positive relationship. Your own dialysis nurse is our frontline support administering your treatment and additional support is provided by our bio-med technicians for equipment maintenance; Registered Dietician and Master Social Worker to work on your personal needs and concerns.
